► di essere consapevole che la mia condotta dovrà essere irreprensibile sotto gli aspetti morale, civile e sportivo;
► di non essere stato precedentemente allontanato, radiato e di non aver subito gravi sanzioni disciplinari presso altri circoli sportivi;
► di aver preso visione e di accettare in ogni sua parte il Regolamento della Società Sportiva e lo Statuto Sociale e le norme di sicurezza ;
► di aver preso visione delle quote di partecipazione praticate dalla S.S.D. Kite Beach Fiumara per i propri corsi e che mi impegno ad onorare e rispettare;
► di essere consapevole che la durata del tesseramento e dell’iscrizione alla Società Sportiva Dilettantistica Kite Beach Fiumara è stabilita in un anno e viene a cessare automaticamente al 30 Settembre di ogni anno;
► di aver ricevuto, ai sensi del Regolamento (UE) 2016/679 (“GDPR”) articoli 13 e 14 e successive norme nazionali di adeguamento, l’informativa sul trattamento dei dati personali (resa pubblica anche nel sito web della società, www.kitebeachfiumara.it) e di manifestare il mio consenso al trattamento e comunicazione dei miei dati personali, comuni e sensibili, nel rispetto delle finalità e modalità riportate nell’informativa stessa;
► di manifestare il mio consenso alla pubblicazione sul web e/o su materiale cartaceo del mio nominativo e di fotografie od immagini che mi ritraggono realizzate nel corso di lezioni, serate, eventi e/o manifestazioni sportive;
► di essere consapevole delle responsabilità e delle pene stabilite dalla Legge per le false attestazioni e mendaci dichiarazioni (art. 76 D.P.R. 445/00) pertanto, sotto la mia responsabilità, garantisco che i dati sopra riportati sono veritieri.
Dichiaro, altresì, di essere a conoscenza dell’obbligo del certificato medico d’idoneità alla pratica sportiva non agonistica e come richiesto da regolamento, mi impegno a presentar lo al più presto alla Società Sportiva Dilettantistica.
Nel frattempo dichiaro di assumermi ogni responsabilità nel caso dovessero verificarsi infortuni o malori alla mia persona causati dalla mia imprudenza, negligenza o imperizia.

DECLARE

I am aware that my conduct must be beyond reproach under the moral aspects, civil and sport;
► not been previously dismissed, disbarred and had not suffered serious disciplinary action at other sporting clubs;
► to have read and accept in its entirety the Sports Club Rules and the Articles of Association and the safety rules ;
► to have taken vision of the participation fees charged by S.S.D. Kite Beach Fiumara for their courses and that I pledge to honor and respect;
► be aware that the term of membership and enrollment in the Company Amateur Sports Kite Beach Fiumara is established in a year and comes to an end automatically on 30 September each year;
► have received, pursuant to Section 13 Legislative Decree n. 196/2003, the information on the processing of personal data (also made public on the website of the company, www.kitebeachfiumara.it) and express my consent to collect and process my personal data, common and sensitive, in compliance with the purposes and methods described in the information;
► to express my consent to the publication on the web and / or paper material of my name and photographs or images that portray me made during lessons, parties, events and / or sports events;
► to be aware of the responsibilities and of the penalties established by law for false claims and false statements (art. 76 of Presidential Decree 445/00) Therefore, under my responsibility, I guarantee that the above data are true.
I declare, also, to be aware of the obligation of the medical certificate of fitness to practice non competitive sports and as required by regulation, I pledge to him as soon presentar the Company SSD.
Meanwhile hereby assume all responsibility if they occur injuries or illness to my person caused by my recklessness, negligence or incompetence.
